Position Summary

Supervisor: Clinical Services Team Coordinator/Supervisor

GOAL: To provide high quality, trauma-informed & responsive therapeutic services, case management and support to Gracehaven treatment team, clients & their families/support teams, utilizing evidence informed, evidence based therapies & treatment modalities.

KEY OBJECTIVES

  • Provide individual and family-based trauma-informed clinical services to youth who are at risk and/or have been exploited and are active clients engaged in Gracehaven Clinical Services, with a primary focus on our Community Based Services.
  • Facilitate and promote family engagement and collaborative partnerships with community stakeholders
  • Complete a Comprehensive Diagnostic Assessment and/or Update for all new clinical services clients upon intake and update annually.
  • Provide level of care and treatment recommendations based on the Diagnostic Assessment and medical necessity.
  • Oversee, review, and assist with the ongoing development and implementation of the clients individualized treatment plans (ITPs).
  • Provide clinical consultation and support to the Qualified Mental Health Service (QMHS) providers.
  • Contribute to and foster a trauma-informed & responsive organizational culture.
  • Collaboration with the clinical services team with implementing a trauma informed continuum of care and clinical services throughout Gracehaven.
  • Maintain all compliance standards.
  • Maintain spiritual integrity and leadership
  • Actively participate in supervision & ongoing professional development- training to foster the development of clinical skills, provide best practice and enhance Gracehaven therapeutic services for youth & families who are at risk and/or have been exploited.

SPECIFIC DUTIES

Responsibilities include, but are not limited to, the following:

Clinical Responsibilities:

  • Competency: Establish therapeutic rapport and authentic, trauma-informed relationships with Gracehaven Community Based Services clients and families.
  • Screening & Intake: Assist Supervising Case Manager with reviewing and screening referrals. Work in conjunction with the Supervising Case Manager to facilitate the onboarding and intake of new clinical services clients.
  • Diagnostic Assessment: Responsible for developing a comprehensive clinical formulation based on the completion of a diagnostic assessment, to establish medical necessity for services at time of admission. The comprehensive assessment will include at minimum an interview with the client, review of records, observation of current level of functioning, the formulation of a diagnostic impression, level of care, and individualized treatment recommendations.
  • Treatment Planning: Facilitate the collaborative development of an Individual - Treatment Plan (ITP) with the client, family, and other service providers. Monitor implementation of this plan. Regularly review, update/revise service recommendations/treatment goals, intensity of services based on the ongoing assessment of client’s progress and evolving needs. Ensure the ITP continues to meet criteria for medical necessity of this plan with the interdisciplinary team according to regulatory guidelines (i.e. OhioMHAS, Medicaid, etc.)..
  • Clinical Consultation: Provide Clinical Consultation to the QMHSs regarding Therapeutic Behavioral Services and Community Psychiatric Supportive Treatment (CPST).
  • Clinical/Case Management Services: Provide psychotherapy, therapy groups, therapeutic intervention services, and/or CPST services as caseload permits. Facilitate problem solving strategies when/if obstacles and/or challenges arise with their assigned clients. Provide prevention education to youth and families on topics relevant to their needs utilizing best practice curriculum (DMST education curriculum, stages of change, pregnancy prevention, relapse prevention, etc.)
  • Discharge Planning: Participate in quarterly Multidisciplinary Progress Review and Maintenance/Discharge Planning meetings and serve as an advocate for the clients to ensure that appropriate level of care and services are being provided in the least restrictive environment necessary. Communicates changes in recommendations and/or referrals in a timely and concise manner to all stakeholders.
  • Documentation: Provide accurate and timely documentation of service delivery and client’s progress, in accordance with the treatment goals and within the timelines set forth by the agency and regulatory guidelines.

Communication & Collaboration:

  • Serves as the point of contact for service team members and families.
  • Responsible for communicating and maintaining good working relationships with the client's family and custodians.
  • Collaborate and maintain effective, timely and thorough verbal and written communication with clients, families, Gracehaven service team, and other interdisciplinary service team members pertinent to each client’s formal/informal support system.
  • Work in conjunction with the client and team to identify needs and connect them to community-based resources and services (i.e. medical services, social service, access to tangible needs) to remove barriers to access to care (i.e. transportation, housing, etc.) and meet basic needs.
  • Facilitate mentoring relationships in partnership with COYFC Mentoring by referring clients for mentoring programs when appropriate.
  • Responsible for family engagement, including communicating and maintaining good working relationships with custodians and partner agencies/organizations working with clients.
  • Responsible for facilitating interagency collaborative meetings, attending and providing advocacy, support, and progress reports to court, attending and participating in children services semi-annual reviews, etc.
  • Actively participates in a coverage rotation for monthly coalitions & sub-committee meetings, safe harbor court(s), and specialized initiatives (i.e. PATH, etc.).

Crisis Intervention Services:

  • Provide crisis management consultation to Gracehaven Community Based Team,
  • Complete ongoing client Mental Status Exams (MSE) and Risk Assessments and when screeners warrant an emergent need for a Risk Assessment.
  • Identify clients with urgent/emergent mental health symptoms (e.g. suicidal, homicidal, psychosis, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities when appropriate.
  • Assist the client, their family, and the Gracehaven Clinical Service Team in developing effective safety and/or behavioral intervention plans.
  • Participate in a monthly Clinical On-Call rotation.

Trauma Informed-Responsive Organizational Culture Requirements:

  • Supports the philosophy, goals, and mission of Gracehaven.
  • Collaboration with the clinical services team with implementing a trauma informed continuum of care and clinical services throughout Gracehaven.
  • Share information and training resources with staff that support the implementation of trauma informed/responsive practices when interacting with clients, families, and service team members.
  • Supports a trauma informed and responsive team ethos by modeling, coaching, and/or reinforcing the teaching of symptom management, including relaxation techniques, anger management, therapeutic goal-setting, problem solving, coping skills, etc.
  • Utilizes therapeutic, trauma informed/responsive intervention techniques before, during and after times of crisis to support safe client de-escalation and stabilization.

Quality Assurance and Compliance Responsibilities

  • Maintain an active license that is in good standing with the state of Ohio Social Work and Counselors Board.
  • Obtain and maintain Dialectical Behavioral Therapy Certification, Gracehaven Level II QRTP approved Trauma Model.
  • Participate in professional development (e.g. training, workshops, conferences, etc.) to maintain professional knowledge and proficiency, as well as the Continuing Education Units (CEUs) necessary to maintain appropriate licensure and credentialing.
  • Clinical services must be provided in accordance with all Gracehaven policies and procedures, as well as all current federal, state, and local standards, guidelines, and regulations governing Gracehaven Therapeutic Services. This includes, but is not limited to providing therapeutic services in a manner that ensures that the confidentiality of the client is maintained in accordance with federal HIPAA regulations.
  • Maintain accurate and timely documentation/outcomes data/records and service delivery requirements, as set forth by the agency policy & procedures and OhioMHAS.
  • Meet monthly productivity standards for the position.
  • Participate in program quality improvement activities and maintain quality assurance standards.
  • Strive to maintain positive client and stakeholder satisfaction.
  • Responsible for maintaining accurate and up to date case management records, case notes, and program documentation in accordance with the standards specific to CARF, OhioMHAS, OAC codes, ODJFS, HCGC/Medicaid, Grant requirements, Contracts DCFS, ODYS, and other regulatory bodies.
  • Complete all required documentation utilizing all required systems and submission processes, within 48 hours (2 business days) or within the specified deadline.
  • Document and report mileage & expenses monthly for accountability and financial stewardship.
  • Report monthly data in accordance with grant requirements and Gracehaven quality assurance/insurance data collection requirements.
  • Comply with applicable state and federal laws and regulations relating to the security, protection and privacy of Personally Identifiable Information (PII) and Protected Health Information (PHI), including without limitation the Health Insurance Portability and Accountability Act of 1996 and regulations as amended and updated.

Supervision/Team Responsibilities

  • Attend COYFC/Gracehaven All Staff training and staff meetings.
  • Assist in all Gracehaven/COYFC Fundraising Events & Development Team initiatives (Over the Edge, See the Story, Be the Story, etc.) Invite 4 new people to See the Story/Be the Story each year.)
  • Actively participate in supervision, community team meetings, case reviews, and other intra-agency meetings.
  • Work in conjunction with Gracehaven Director of Clinical Services & Community Based Programming to identify and implement an evidence based treatment model for Community Based Treatment Services.
  • Responsible for actively participating in bi-weekly individual resilience informed practice supervision, resilience based group supervision and team building initiatives, and weekly/bi-weekly case consultation/staffing.
  • Responsible for creating and implementing an individual professional development plan in conjunction with your direct supervisor.
  • Responsible for attending 20 hours of training within the first 30 days, and 32 additional hours within the first year of employment (52 hours of training within the first 12 months) and participating in 24 hours of additional ongoing training and professional development opportunities each year following.
  • Participate in at least 10 of the 12 monthly Gracehaven All Staff Meetings & 3 out of 4 COYFC All-Staff meetings.
  • If CHW licensed, attend at least 9 of the 12 monthly CHW meetings and all scheduled mandatory training, and conference calls. If not able to attend a meeting, contact HCGC Pathways HUB to notify and review missed meeting information.

Position Requirements

Minimum Qualifications:

  • Demonstrated skills in managing care to children and families, with experience in providing care to individuals impacted by trauma, especially sexual violence and trafficking, and their families.
  • Excellent communication or interpersonal skills to establish and maintain positive relationships with clients, their families, and multidisciplinary professionals/service providers.
  • Education: Master’s degree in social work/counseling or a related behavioral sciences field from an accredited university or professional school required. Note: MSW and LPC Interns under the direct supervision of an LISW-S or LPCC-S may be considered for this role.
  • Licensure: LSW or LPC issued from the State of Ohio Counseling and Social Worker Board, or is license eligible and willing to obtain, required; LISW or LPCC (Independent License) preferred; LISW-S or LPCC-S (Supervisory License).
  • Certifications: Level II or III QRTP approved Trauma Model certified is preferred; or willingness to obtain required.
  • Certifications: Ohio Comprehensive Child and Adolescent Needs & Strengths (CANS) Certified or willing to obtain certification.
  • Transportation: A valid Ohio Driver’s Licenses, Automobile Insurance that meets minimum requirements, and a safe driving record.
  • Personal vehicle, reliable transportation that meets Ohio Highway Patrol Inspection Standards.
  • Experience: Two (2) or more years providing trauma informed counseling to youth and families is preferred.
  • Clean background check
  • Agreement with the Central Ohio Youth for Christ and Gracehaven Standards of Conduct and Statement of Faith.
