Position Summary

GOAL: The Clinical Coordinator role is a unique blend of therapeutic services and administrative oversight of regulatory requirements. We are seeking a female therapist who is empathetic, highly competent, detail-oriented, excels in time management, and feels a genuine calling to serve minor victims of sex trafficking.

 

KEY OBJECTIVES

     This position provides both clinical leadership and direct therapeutic care.

     The Clinical Coordinator offers clinical supervision and training to direct care staff and

oversees all employees involved in providing clinical services.

     The role includes providing individual, family, and group counseling to residents and their families/care givers, as well as trauma-informed support and secondary trauma care to residential and community-based staff.

     With a balance of compassion, professionalism, and strong administrative skills, the Clinical Coordinator ensures that clinical practices meet the highest standards while fostering healing and resilience for both clients and staff.

     Proactively contribute to and foster a trauma-informed & responsive organizational culture.

     Maintain all compliance standards.

     Actively participate in supervision & ongoing professional development- training to foster the development of clinical skills, provide best practice and enhance Gracehaven therapeutic services for youth & families who are at risk and/or have been exploited.

 

SPECIFIC DUTIES

Responsibilities include, but are not limited to, the following:

 

Clinical Responsibilities:

     Referrals: Assist in reviewing Group Home referrals (i.e. ICCAs, clinical assessments, Ohio Comprehensive CANS or other LOC tools, etc.) and providing clinical placement consultation to the Residential Services Director. This includes conducting pre-placement interviews as needed, as well as facilitating intakes for placement.

     Establish therapeutic rapport and relationships with Gracehaven Group Home clients and families.

     Assessment: Responsible for developing a comprehensive clinical formulation based on the completion of a diagnostic assessment, to establish medical necessity for services at time of admission for group home residents and community clients.

     Treatment Planning: Participate and contribute to the collaborative development of an Individual Service (ISP)/Treatment Plan(ITP) with the client, family, and other service providers. Monitor implementation of this plan. Regularly review, update/revise service recommendations/treatment goals, intensity of services based on the ongoing assessment of clients progress and needs, and maintain medical necessity of this plan with the interdisciplinary team according to regulatory guidelines (ODCY-QRTP, OhioBHS, etc.). Review and provide approval for (sign off on) residents Individualized Service Plans/Case Plans for contraindications and quality assurance, including compliance with all DCY licensing requirements.

     Counseling: Responsible for providing individual, group, and family counseling to group home and community clients and their families based on mutually agreed upon goals with the client and/or parents as identified in the ISP - Treatment Plan. This includes but is not limited to providing individual counseling to clients at least 1 time per week; facilitating Dialectical Behavioral Therapy (DBT) group 1 time per week.

     Behavioral Intervention: Assist the client and Gracehaven service team in developing effective behavioral intervention plans as needed. Provide evaluation and approval of Behavioral Intervention Plans within 30 days of admission and at minimum every 90 days following, or as frequently as the need necessitates.

     Discharge Planning: Participate in monthly Multidisciplinary Progress Review and Discharge Planning Meetings and serve as an advocate for the clients to ensure that appropriate services are provided and permanency/maintenance planning is prioritized.

     Documentation: Provide accurate and timely documentation of service delivery and client’s progress, in accordance with the treatment goals and within the timelines set forth by the agency and regulatory guidelines.

     Communication & Collaboration: Collaborate and maintain effective, timely and thorough verbal and written communication with clients, families, referring agencies, and other service providers as described in the interdisciplinary team process. Communicate with staff regarding therapy sessions, resident progress, therapeutic homework, prior placements, and new relevant information. Attend county service provider meetings and maintain collaborative relationships with partner agencies.

     Crisis Intervention Services: Provide crisis management consultation to Gracehaven Group Home Team, risk assessments & crisis intervention services to clients and families when deemed necessary. Identify clients with urgent/emergent mental health symptoms (e.g. suicidal, homicidal, psychosis, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities when appropriate. Assist the client, their family, and the Gracehaven Clinical Service Team in developing effective safety and/or behavioral intervention plans.

     Clinical On-Call: Participate in a clinical on-call rotation to provide 24/7 hour clinical crisis management services for active group home clients, including in person and/or telehealth (as appropriate) risk screening and assessment (e.g. suicidal, homicidal, psychotic, etc.) and crisis intervention services. Identify clients with urgent/emergent mental health conditions (e.g. suicidal, homicidal, psychotic, etc.) and make appropriate referrals to available emergency or psychiatric treatment facilities when appropriate.

     Assist the client and Gracehaven service team in developing and implementing effective safety plans as needed.

     Provide accompaniment, advocacy, and testimony, as needed on behalf of clients who are court involved. 

Position Requirements

Trauma Informed-Responsive Organizational Culture Requirements:

     Work in conjunction with Gracehaven Director of Clinical Services to implement and maintain QRTP evidence based treatment model & therapeutic interventions services.

     Demonstrates personal and professional accountability and supports the philosophy, goals, and mission of Gracehaven.

     Shares information and training resources with staff that support the implementation of trauma informed/responsive practices when interacting with clients, families, and service team members.

     Design, develop, publish, or approve trauma-informed group counseling curricula and interventions for individual counseling.

     Supports a trauma informed and responsive ethos at the group home by modeling, coaching, and/or reinforcing the teaching of symptom management, including relaxation techniques, anger management, therapeutic goal-setting, problem solving, coping skills, etc.

     Utilizes therapeutic, trauma informed/responsive intervention techniques before, during and after times of crisis to support safe client de-escalation and stabilization.

 

Compliance Responsibilities

     Maintain an active license that is in good standing with the state of Ohio Social Work and

     Counselors Board.

     Obtains and maintains Dialectical Behavioral Therapy Certification, Gracehaven Level II QRTP approved Trauma Model.

     Ensure compliance with all placement contracts and the expectations set forth in them. Read, understand, and comply with contracts from each county and funding source.

     Participates in professional development (e.g. training, workshops, conferences, etc.) to maintain professional knowledge and proficiency, as well as the Continuing Education Units (CEUs) necessary to maintain appropriate licensure and credentialing.

     Therapeutic services must be provided in accordance with all policies and procedures, and current federal, state, and local standards, guidelines, and regulations governing Gracehaven Clinical Services.

     Provide therapeutic services in a manner that ensures confidentiality of the client is maintained and HIPAA guidelines are followed.

     Maintains accurate and timely documentation/outcomes data/records and service delivery requirements, as set forth by the organizational policy & procedures, as well as regulatory bodies.

     Meet the monthly productivity standard for the role, as outlined by organizational policy.

     Participates in program quality improvement activities and maintains quality assurance standards.

     Strives to maintain positive client and stakeholder satisfaction.

Supervision/Interdisciplinary Team Collaboration Responsibilities:

     Provide clinical supervision to group home and clinical team staff.

     Conduct, review, and troubleshoot clinical issues as they arise.

     Oversee and facilitate weekly treatment team meetings/staffings.

     Meet with group home staff to discuss clinical practices within the direct care role (weekly/quarterly).

     Foster a staff environment that supports self-care practices.

     Provide support, resources, and debriefing for group home-staff experiencing secondary trauma; remain attentive to staff needs and stressors.

     Maintain accessibility and/or on-site presence at the residence 32 hours per week.

     Attends all COYFC/Gracehaven training and staff meetings.

     Assists in all Gracehaven Fundraising Events.

     Participates in individual and group supervision and other intra-agency meetings.

Minimum Qualifications:

     Demonstrated skills in managing care to children and families, with experience in providing care to individuals impacted by trauma, especially sexual violence and trafficking, and their families.

     Excellent communication or interpersonal skills to establish and maintain positive relationships with clients, their families, and multidisciplinary professionals/service providers.

     Education: Master’s degree in social work/counseling or a related behavioral sciences field from an accredited university or professional school required.

     Licensure: MSW-LSW or LPC issued from the State of Ohio Counseling and Social Worker Board required; LISW or LPCC (Independent License) preferred and willing to obtain LISW-S or LPCC-S (Supervisory License) preferred.

     Certifications: Level II or III QRTP approved Trauma Model certified is preferred; or willingness to obtain is required.

     Certification: Ohio Comprehensive Child and Adolescent Needs & Strengths (CANS) Certified or willing to obtain certification.

     Experience: Two (2) or more years providing trauma informed counseling to youth and families is preferred.

     Clear background check

     Active Ohio Drivers License, in good standing.
